Incident Reports

In the acknowledgement section of the Incident Report, the signature field currently listed as "Notifier" should be changed to "Director/Responsible Person."

It is essential that the Director or Responsible Person in charge on the day is informed of all incidents that occur at the service. This ensures appropriate oversight, follow-up actions, and compliance with regulatory requirements.

As incident records are legal documents that must be retained until the child reaches 25 years of age, the person responsible for the operation of the service on that day must be aware of every incident and acknowledge that they have reviewed the report.

The acknowledgement section should therefore include:

Educator: Yes

Witness: Yes (if applicable)

Director/Responsible Person: Yes

This change ensures accountability, accurate record-keeping, and compliance with legislative and regulatory requirements.
